BDSG
member organisations encourage divers to report any incidents that occur
so that an analysis can take place and others can learn from what occurred.
Any personal information is treated with strict confidence. Divers can
submit such reports through their own training organisation or directly
using a form available for download below. The BSAC compiles a central
database with the assistance of other BDSG members. Incident information
and statistics can be readily accessed at http://www.bsac.org/page/544/diving-incidents.htm.

If
the incident took place while you were at work, you may also have a
legal duty to report it to the Health and Safety Executive. Further
information, and an online reporting facility can be found at http://www.riddor.gov.uk.
It
is important that equipment used in an incident is properly handled.
